I hereby absolve Hockeytown USA, it's owners and instructors from all liability for injuries to these registered persons during these sessions. I hereby attest that the applicant is in good health and able to participate in physical activity of a vigorous athletic activity. It is also my decision to have my child wear a helmet with or without a face mask. I understand that helmets with face masks are highly recommended by Hockeytown USA and it's instructors.
